Thyroid Optimization
Treatment Overview
What thyroid optimization actually looks like
Most patients who come to us for thyroid care have already been told their labs are normal. They have a TSH in range and a doctor who has moved on. And yet they are exhausted, gaining weight, losing hair, cold when no one else is cold, and struggling to think clearly. The labs said fine. The body is saying otherwise.
The issue is that TSH alone tells you almost nothing about how well your thyroid is actually functioning. A complete evaluation includes Free T3, Free T4, Reverse T3, and thyroid antibodies. Most conventional workups skip most of these. Dr. Apostol runs the full panel, interprets it against optimal ranges, and addresses the actual dysfunction, whether that is a conversion problem, Hashimoto’s, nutrient deficiencies, or medication that has never been truly dialed in for your physiology.
"I have seen patients who spent years on thyroid medication that was not working for them. Not because the diagnosis was wrong, but because the treatment was never truly optimized."
— Dr. Margaret Apostol, MD

Treating Your Thyroid The Right Way
Why patients come to us
for thyroid care
Most patients arrive having already been told their thyroid is fine, or having been on medication for years without ever feeling fully well. They are looking for someone who will actually look closer.
You have been told your labs are normal
A TSH in range is not the same as a thyroid that is working well. We run a complete panel and read it against optimal ranges, not just the broad thresholds that most labs use.
You are on medication but still feel off
Many patients on thyroid medication never feel fully well because the medication type or dose was never truly optimized for their physiology. There is more to get right than just a prescription.
You have Hashimoto's and want real answers
Hashimoto's is an autoimmune condition that requires more than just thyroid hormone replacement. Dr. Apostol investigates the immune drivers and addresses them, not just the downstream hormone levels.
Your symptoms have never been fully explained
Fatigue, weight gain, brain fog, hair loss. These have been attributed to everything except what may actually be driving them. A thorough thyroid workup is often where the answers start showing up.

Frequently Asked
Questions
Normal on a standard TSH test means you have not crossed the threshold for a clinical diagnosis. It does not mean your thyroid is functioning optimally. Many patients feel significantly better once their Free T3, Free T4, and other markers are brought into truly optimal ranges, even when TSH was technically within the reference range.
We run a comprehensive panel including TSH, Free T3, Free T4, Reverse T3, TPO antibodies, and thyroglobulin antibodies. This gives us a complete picture of thyroid hormone production, conversion, utilization, and whether any autoimmune activity is present. Most conventional workups only include TSH.
Yes, and this is one of the most common reasons patients come to us. Many people on thyroid medication never feel fully well because the medication type, dose, or timing was never truly optimized. We review the full picture and make adjustments based on both your labs and how you actually feel.
Hashimoto's thyroiditis is an autoimmune condition where the immune system attacks thyroid tissue. Conventional treatment typically only addresses the resulting low thyroid hormone. At Apostol IFM we also investigate and address the immune drivers, including gut health, nutrient deficiencies, and food sensitivities that contribute to autoimmune activity.
Dr. Apostol prescribes based on what your labs and symptoms indicate you actually need. Some patients do well on T4 alone. Others convert T4 to T3 poorly and do significantly better with combination therapy or desiccated thyroid. The goal is to find what works for your specific physiology, not apply the same protocol to everyone.
It varies. Some patients notice improvements within a few weeks of starting or adjusting treatment. Others take several months as dosing is refined and the body adjusts. Thyroid optimization is an iterative process and Dr. Apostol will set realistic expectations based on where you are starting from.
Yes. Thyroid hormone directly regulates metabolism. When thyroid function is suboptimal, the metabolic slowdown can make weight gain nearly inevitable and weight loss almost impossible regardless of diet and exercise. This is one of the most frustrating aspects of undiagnosed or undertreated thyroid dysfunction and one of the clearest reasons to get a complete workup.
Our thyroid and hormone therapy services are offered on a private pay basis and are not billed through insurance. Many patients find that investing in personalized, cash-pay care gives them faster access to treatment and more flexibility in their options. We offer transparent pricing and are happy to discuss payment options during your consultation.
